Chamberlin discusses the issue with the mental health profession creating a community of mental health centers in which the patient still goes unnoticed. She says that "for meaningful public debate about the rights of present and former patients to occur, the missing voice of the patient must be added to those of other recognized interest groups." Ex-patient groups are the only ones qualified to speaks as representatives of patients, but their groups are often small and poor. Chamberlin argues that the development of strong ex-patient groups is necessary to ensure that patients are represented in discussions of community mental health centers. Chamberlin has made numerous handwritten edits to the paper.
